Policies

The therapists, team and associates of Align’d Body Mind and Reviveskin Medical Spa are committed to providing you with relief, recovery, rehabilitation, relaxation and rejuvenation for every guest and client who walks through our door. 

Prior to your visit please read through our office policies. All appointments are subject to these policies and we thank you for your understanding and cooperation. If you have any questions please call or email the office and we will happily discuss any specific concerns and needs. 

Scheduling Appointments

Jane makes it ridiculously simple for patients to book their own clinic appointments online. With a few clicks of your mouse, you can see which time slots are available for every service and therapist and add one or more appointments — in real time — to your clinic’s schedule. There is even a waitlist you can sign up for so you are the first notified for a cancellation spot.

If you prefer the personal approach, please feel free to email our team at team@alignd.ca or call us at 905-749-0304. Our front desk team is available from 10 am to 7 pm Monday to Friday to answer or return your emails, texts and calls.

For our virtual Maxim Naturopathic Appointments, offered Tuesday through Saturday, please send an email request to team@alignd.ca or call the office (905-749-0304) and we’ll get you booked in.

Cancellation Policy

Please be mindful of our strict and equitable cancellation policy and inform us right away if you are not feeling well or need to change or cancel your appointment. 

We will be happy to accommodate any appointment cancellations or rescheduling by 6pm the day before your appointment without any charge. 

A strict 50% fee will be applied to appointments cancelled after that time until 2 hours before your booked time

After that time a 100% fee will be applied to missed or late arrival appointments. Thank you for respecting our policy and notifying us ASAP when you need to cancel or change times as this helps you and others on our waitlist to be seen sooner. Please email team@alignd.ca or call 1-905-749-0304.

*Winter storms with adverse driving conditions and TRUE emergencies are the only exception with advance notice provided to the clinic* We will send an email or text if our therapists are unable to make it to the clinic during winter storms. 

Payment Options

Align’d Body Mind Inc. accepts cash, e-transfers or credit cards (MasterCard, Visa and American Express) for services rendered. 

We require a credit card to be held on file for:  Virtual Naturopathic Appointments, Saturday RMT appointments; clients who have missed appointments without notice; and if you want to use insurance direct billing (as portals can be in maintenance for any remaining amounts not covered.)

You can add your credit card to our Jane booking app securely. It uses Stripe which has the highest security policies.  No charges will be applied to your credit card without notice or explanation and unless you miss or cancel an appointment without proper notice, as per our cancellation policy. Payment is always due at the time of service to remain compliant with the RHPA (Regulated Health Professions Act).

A receipt will be issued for every service. It can be emailed or printed for you. Although many health services are HST exempt (chiro, acu, physio, naturopath) applicable taxes will be applied to services who must charge HST (RMT, Osteopathy, Live Blood Analysis, Psychotherapy, Medical Spa).

Insurance Information

Align’d Body Mind Inc. accepts a wide range of insurance plans. Most Regulated Health Professional services are covered by many insurance plans and we will gladly bill your insurance company for reimbursement for those services for you, if coverage is available. Please see our direct billing policy.

Some services (PEMF and Live Blood Analysis as a standalone treatment) are not covered by insurance, we can not bill your insurance for these services, nor are services from Reviveskin Medical Spa. At this time, direct billing is also unavailable for Osteopathy although this should become available by 2024. These services therefore cannot be submitted for reimbursement and will require any payment due on the day of service. 

Medical Forms & Records

Please take a moment to fill out our mandatory and confidential online intake form before your visit. If you prefer to do a written one please come 15 minutes early and our receptionist can provide one for you. The Regulated Health Professions Act (RHPA) and insurance providers require all clients to complete and sign a Health History Form for each health service (acupuncture, RMT) BEFORE the treatment commences.

As per the RHPA, all health intake forms must be able to identify the client. A name, address, primary care physician information and relevant medical history including past and current conditions and medications are required legally for every client. For legal and liability purposes we cannot provide treatment or sessions without this information. It also must be updated for every service type (not therapist) once a year.

Your personal information is private, confidential and protected by Align’d Body Mind and our affiliates including staff, our Jane System, financial records and payment processing software. We strictly enforce and follow protocols and requirements set and supported by PIPEDA (Personal Information Protection and Electronic Documents Act), PHIPA (Personal Health Information Protection Act) and RHPA (Regulated Health Professionals Act). All regulated healthcare professionals must obtain and record patient consent before disclosing a patient’s personal health information to someone who is not a health provider involved in the patient’s care.
